Performing a physician search on CertiFACTS On-Line is a simple process.
First Step:
Click the "login button" located in the navigation panel on the left hand side of the page. Enter your CertiFACTS On-Line username and password and click "go".

Second Step:
Select either the "basic" or "advanced" search. The advanced search allows you to enter only a social security number if you so desire.

A screen shot of the basic search page is listed below. The system will do a "like" or wildcard search on the names so hyphens do not need to be included. Either or both of the elements can be input. We recommend either a last name or first name or a social security number in the advanced search.

A screen shot of the advanced search page is listed below. As you can see, there are a number of additional categories listed that will allow you to perform a more detailed search. There are also help icons listed by each category should you require a detailed explanation. Only one search element needs to be selected and partial entries can be made, i.e. first letter of the last name or first few digits of the social security number. However, the less refined the search, the more results will display. We have social security numbers for the majority of the providers but some are missing. Therefore, if no result appears try searching by using another data element.
Third Step:
Upon entering your search criteria and clicking the "search" button you will be transferred to the "search results" page where you can select the provider from the results to view the official data report. You can also refine the search if there were a large number of names listed from your first attempt. You select a name by clicking on the ABMSUID or by clicking the box to the left and selecting "report" from the menu options at the top of the search results. Multiple names may be tagged and a batch report generated that will print individual profiles with one or two clicks. Very handy during reappointment.
Fourth Step:
Confirm the information listed on the report and print a copy for your files.
